Does Corr replace Home Health?

No. Corr complements your existing providers with ongoing coaching, monitoring, and care coordination. It does not replace emergency or in-person medical care.

How often do you check on me?

Your dedicated health coach checks in weekly, or more often when your care plan and health needs call for it.

What happens if something changes?

Your care team reviews check-ins and connected-device trends, follows up with you, and coordinates the appropriate next steps with your providers.

Who sees my information?

Only authorized members of your Corr care team and providers involved in your care, in accordance with Corr’s privacy practices.
